There is an extremely annoying "feature" where the app repeatedly prompts me (ten times?) for my smart card credentials regardless of if my card reader is connected to the system or not. If this is only happening in Xbox app then try first resetting it at Settings > Apps > Apps & Features > Xbox Live > Advanced Options > Reset. Restart PC, if it persists then go back to same place to Uninstall app. Restart PC, go into Microsoft Store settings > My Library to reinstall the app.
